☆ ＨＩＶ／エイズの実態は？(１) ――――――――― 2004/12/15
１２月１日は世界エイズデーでした。だから、それに関する記事で盛りだくさ
ん。中国と日本、世界のエイズ事情について資料を集めました。世界のエイズ
感染者は４千万人前後、その９０％以上が途上国で発生。
中国のＨＩＶ感染者は８４万人いますが桁が違う！アフリカはもっと多く、特
にサハラ砂漠以南では２５００万人以上。中国では、売血によって感染した人
が多かったらしい。麻薬中毒患者の注射針の回しうちも感染経路の一つです。
売春行為はなくならないが、コンドームが大衆に普及している日本は大丈夫。
女性もセックスを拒むことができる社会だと思います。しかし売春行為で感染
した夫が妻にＨＩＶを移すというケースが珍しくないのは確かです。
――まずオーストラリア留学中の女性の話を引用します。
「オーストラリアでは、麻薬中毒者は絶対にあとを絶たないし、彼らが完全に
麻薬を止めることは不可能だと判断されている。そこで、麻薬中毒者を取り締
まるよりも、彼らが安全に麻薬を打てる環境を整えるべきだ、との考えから、
「Safe Injecting Room」が設立されています。
ここでは、初回訪問者に新しい注射器を与え、次回訪問からは格安で注射器を
中毒者へ販売します。薬自体は、麻薬中毒者独自で入手する必要があり、麻薬
を持参した中毒者に、清潔な環境で麻薬を打てるように手助けをしているそう
です。
施設内には、シャワールーム、洗濯機や乾燥機、カフェやプレイルームなどが
用意されています。
実際に麻薬を打つ「Safe Injecting Room 」ではスタッフが立ち会い、中毒者
の「fatal overdose」麻薬過剰摂取による死亡を防ぐ努力をしています。
スタッフは中毒者を見守るだけで、スタッフが中毒者に麻薬を打つことはない
そうです。
駅のトイレや大きなショッピングセンターのトイレに、麻薬中毒者が使用した
注射器を捨てるための専用のごみ箱が取り付けてある。箱の上部に直径２〜３
センチの、穴のあいた黄色い箱が、トイレの手洗い場、もしくは個室１つ１つ
に設置してあったりします。
この箱がないと中毒者達は使用済みの注射器を簡単に道端などに捨て去るそう
です。通行人が道で転んでその針で怪我をしただけでなく、それが原因でＨＩ
Ｖに感染するということもあるとか。
だから、この箱や「Safe Injecting Room 」は、一般人の身の安全を考えても
麻薬中毒者のことを考えても、必要な配慮と制度、または施設だということで
す。
映画館で座席に座る時には、席をよくチェックしてから座るように！
ＨＩＶに感染した麻薬中毒者の使用済み注射針が座席に隠されていて、そこに
座った客がその針で怪我をして、しかもＨＩＶに感染してしまったのだから！
という話もあります。
―― 次は、
シドニーの風俗店で働きながら、英語の語学研修もこなしている日本人女性の
話。
「すべてのお客さんの８割ぐらいがアジア人です。一番多いのは中国人かな。
オーストラリアは中国人が多く住んでいるので、当たり前といえばそれまでで
すが。
オーラルセックスでエイズになる可能性があると、うちのお店のオーナーから
聞いたことがあります。わたしのお店の女の子は一ヶ月一回、必ずエイズ検査
をします。入店のときもそうです。でもこれだけで安心？
うちのお店は、性行為は基本的にダメですので、女の子もエイズに対してそれ
ほど関心がないようです。正直言って、わたしも関心がない人のひとり。でも
手遅れにならないうちに、真剣に考えていかないとだめな問題だと思います。
売春行為が違法ではない国はたくさんあります。
昔から女性が携わってきた職業です。でも現在のように性感染症、特にＨＩＶ
エイズが蔓延している状況下でこの仕事に従事するのは大変危険なことです。
エイズ検査を月一回しているだけでは、感染は防げません。性行為が禁止だと
しても、それ以外のルートで感染する可能性は十分あります。
タイはゴールデントライアングルといわれるケシの生産地に近いこともあり、
売春行為によってエイズに感染した例がアジアで最も多い国です。最近の対策
として、売春婦は必ず客にコンドームを装着するように要求すること！という
ものがあり、この効果が現れてきています。
しかし、アジアを含めアフリカなどの国では、売春婦から移されたエイズウイ
ルスが、夫から妻へと感染するという経路が後を絶ちません。女性の地位が低
い、エイズに対する認識もなく、予防法も知らないからでしょう。
読み書きのできない女性が多いことも原因の一つです。普通の教育も受けられ
ない状況で、エイズに関する知識を得るのは不可能です。夫に求められるまま
セックスをし感染してしまう。
避妊は一般的には行われていない。ましてコンドームを使用することなど問題
外。望まない妊娠をした場合、中絶するのが一般的。女性の人権が無視されて
います。

ＨＩＶ／ＡＩＤＳ situation (1)
December 1 was “World AIDS Day”, and therefore there were many news
articles and opinions on this issue.
I have collected some of them from China, Japan, Australia and the
world.
HIV/AIDDS infected cases are about 40 million globally, and more than
90% are found in the developing countries.
In China, 840,000 people are HIV carriers.
Africa has a lot more, in particular, south of Sahara Desert reports
25 million.
Many Chinese people were infected with the virus through illegal
blood selling.
Sharing HIV contaminated needle among drug users is another source of
infection.
Buying and selling sex will never die out, but I feel Japanese
situation is relatively all right due to the prevalence of condoms,
Japan is also the society for women to say ‘NO’ to unsafe sex.
However, increasing number of HIV infection is reported in ordinary
families.
After sexual contact with prostitutes, men transmit the virus to
their wives.
A Japanese female studying in Australia is quoted as saying;
“In Australia, it is considered impossible to cramp down on drugs,
and for junkies to stop drug injection.
This is why the government set up ‘Safe Injecting Room’ where drug
addicts inject drugs in a safe and clean atmosphere.
Here the new visitors are given new syringes and on their second
visit and next they can purchase new syringes at a lower price.
Around the facility, there are shower rooms, washers and dryers,
cafes and playrooms.
In the ‘Injection Room’, staff members watch drug users inject
their own drugs to prevent death from overdose.
Why is Australia so generous with drug addicts?
In fact, syringe disposal boxes are placed in station and supermarket
lavatories.
Without such disposal boxes, junkies litter used syringes with
needles on the street.
Passersby could be hurt by the discarded needles and infected with
HIV/AIDS.
At cinemas, people are warned to be seated after careful look if
there are used needles on the seat.”
The following is the story of a Japanese woman working in a sex
industry.
“About 80% customers are from Asia.
Among them, the majority is Chinese.
My manager told me that there is a possibility of contracting HIV
through oral sex.
We are required to take AIDS checkup once a month, however, this does
not seem enough.
We are not allowed to have a real sexual intercourse, and frankly
speaking, all women including me working here have little interest
and awareness of HIV/AIDS infection.
I am beginning to feel it necessary to think about this issue before
it is too late.”
Selling sex is not illegal in many parts of the world.
It is one of the oldest female professions.
However, in the present situation of AIDS prevalence, such body
business is very risky.
Merely taking monthly AIDS checkup is not sufficient.
Although they are not allowed to do real sexual intercourse, they
might be infected with HIV through different channels.
Thailand, due to its proximity to the ‘Golden Triangle’ where opium
poppy is grown, reports the most AIDS infection cases in Asia.
The recent measure to combat HIV/AIDS epidemic is that all
prostitutes should ask their customers to put on condoms.
This tactics seems to bear fruit.
However, including other Asian countries, Africa and elsewhere, the
contagion channels are extended to husband-to-wife sex.
Women’s low social status, lack of knowledge and prevention of AIDS
are attributed to this problem.
When these women lack access to regular schooling, it is impossible
to get proper knowledge of HIV/AIDS.
When wanted, they cannot deny their husbands’ sexual desire and get
infected.
Condom use is out of the question.
In case of unwanted pregnancy, they ordinarily abort babies.
This, I think, is the breach of human right.

エイズは何処かの国が人工的に造った「細菌兵器」ではないか！？という話を
聞いた事があります。どこまで本当か分からない話ですが、冷戦たけなわの頃
の旧ソ連では、ワクチンのない「細菌兵器」が数百トン単位で製造備蓄されて
いたそうですから、あながち根拠のない話でもありません。
それにしても、オーストラリアでは映画館へ行ったら座席に座る前に麻薬中毒
患者が残していった「注射針」に注意しなければならないとは絶句します。
もっとも、最近は日本でも「麻薬」が都市部で流行していて打つ手がないらし
いですから、決して他人事ではないんですよね。
今のところ錠剤状の合成麻薬が主流ですが、そのうちに注射器を使用するタイ
プも出回る事でしょう。
しかし本当に恐ろしいのが此処からで、エイズに感染して思い詰めた人間が、
麻薬に手を出したりすれば大変な事になります。

麻薬を射つための注射針が問題です。廻し射ちというやつで、このために感染
が広まるんです。エイズに感染して思いつめた人間が麻薬に手を出す、のでは
ないと思います。欧米では、「ＨＩＶ／エイズと共に生きる」がスローガンに
なっています。感染したからといって自暴自棄にならなくてもいいのです。
ワクチンもエイズ治療薬もできています。高額ですが、十分免疫力を落とさな
いようにする効果はあがっていますよ。中国でも、欧米で用いられている薬の
ほかに、漢方薬数種類をとりいれる方法が見直されてきています。強い副作用
のある西洋医学の薬よりも、東洋の漢方！というわけです。
ＨＩＶウイルスに感染してもエイズを発病するまでには１０年もかかります。
ですからウイルス検査で「陽性」と診断されても悲観する必要はありません。
免疫力がどんどん落ちていって、他の病気に感染しやすくなる病気ですから、
その免疫力を維持できるように対策をとればいいのです。
そして、他の人にＨＩＶを移さないようにする、本人の責任、モラル。
周りのサポートも必要です。ＨＩＶ感染者が孤立しないように、そして「感染
しているんだ！」と公表できるような社会になってほしいです。ＨＩＶ感染者
には、専門家による心のケアとカウンセリング、家族や友人の励ましが必要だ
と思います。

HIV- infected people should not be desperate and resort to
drugs or other immoral behaviors. There are treatments
available both in western and Chinese medicines. AIDS
vaccine is being developed too. Sharing syringes and
needles among drug users is one of the HIV transmission
channels. Though their drug abuse must be condemned, there
is so-called MMT (Methadone Maintenance Treatment) program
for heroin-dependent patients. The MMT provides addicts
with an option for getting off the needle and sharing
needles, because methadone is taken orally.
There are many HIV carriers in the west who are determined
to live positively with the encouragement from public,
“LIVING WITH HIV/AIDS”
It is said that it takes 10 years for HIV carriers to
develop full-blown AIDS. Hence, there is no need to be
depressed when diagnosed as HIV positive. As AIDS stands
for Acquired Immune Deficiency Syndrome, our immune system
is deteriorated and consequently prone to many symptoms
including lethargy, diarrhea, nausea, fever, coughing and
skin diseases. Treatments are provided to enhance the
immune systems of HIV-infected patients. They do not have
to just wait for death.
Moreover, HIV carriers themselves should be sensible not
to transmit HIV to others. Close friends, colleges and
family members should give emotional support, so that HIV-
victims do not feel isolated or abandoned. Society should
be open enough for them to reveal their HIV infection.
Professional psychiatric counseling is necessary as well.

どうも私の言葉足らずだったようで誤解を招いてしまい、申し訳ありません。
消毒していない注射器の回し打ちがＨＩＶの感染拡大を招くという事は、私も
知っています。
それから、ＨＩＶ感染者が、自棄を起こして麻薬に走るとは、私自身が恐らく
そうするだろうという事なのです。いくら直ぐ死ぬ訳ではないと言われても、
相当な精神的重圧を感じるでしょうし、社会的な「差別」や「偏見」も激しい
事でしょう。ましてや「就職」や「結婚」で影響が無い筈はありません。
自分自身の落ち度でそうなったなら仕方がないですが、もし「薬害エイズ」の
時みたいに、医療ミスで感染したなら誰かを殺しでもしないと収まりが付かな
いですよ。
数日前、東京の病院で、肝臓病にて入院している患者が刃物を振り回して他の
入院患者２名を殺害、女性看護師１名に重傷を負わせるという事件がありまし
た。

確かに甘いです。私の身近には、ＨＩＶエイズ感染者もいませんし、新聞で見
るニュースなどからの知識の受け売りです。
現在、クラスでやっているエイズ関連の討論とコンドーム装着の実演で、中国
の大学生の意識調査をしています。 私と同様、無知な生徒が多いようです。
勉強、試験、宿題、就職と、ずっと追い立てられる生活をしていれば、当然そ
うなるでしょうね。
売買春と麻薬中毒の両面から見ると、これはどう考えても人間の愚かな行為と
しかいえません。だから、その結末でＨＩＶエイズに感染しても、自業自得で
その人は治療を受ける権利もなくしてしまうのでしょうか。社会から総スカン
を食らって自暴自棄になり、エイズを発症して死ぬのを待つだけ？その前に、
自分と同じような苦しみを味あわせてやろうと、だれかれかまわず性行為をし
てＨＩＶウイルスを撒き散らす？麻薬に走る？
その反面、日本の血友病患者が、ＨＩＶに汚染された血液製剤を使って感染し
たケースを考えてみてください。当人の責任ではないから、国や製薬会社、病
院が補償すべき？国民全員が同情する対象になります。

There are no HIV carriers or AIDS patients around me. I
just get information on the newspaper and the Internet.
However, as a teacher, I encourage students to discuss the
HIV/AIDS problems and demonstrate in class how to put on a
condom. Most of them are ignorant of Sex, probably due to
the lack of Sex education and the pressure to study
forgetting ahead.
Selling and buying sex is regarded as one of the foolish
human behaviors.
However, those who are infected with HIV as a result of
their own behaviors should not be entitled to receive a
treatment? Or they are isolated from the society, become
desperate and finally just wait for death? Some would be
anti-social and have sex with anyone to infect them? Or
will they resort to drugs to eliminate sufferings?
On the other hand, Japanese hemophiliac patients
contracted the HIV through blood transfusion or HIV
contaminated blood products. They are the victims and
therefore, the responsibility lies in the pharmaceutical
companies, hospitals and the government. General public
are all sympathetic with them.

さて、エイズの実情はもっと悪化していると思います。勿論地域的にも年齢層
的にも急速に拡大しているとみられます。それが中国政府の重い腰を突き動か
し始めているというべきかと思います。
特に性産業の発展(？)振りは目覚しいものがあります。かっては日陰もの扱い
であったこの手の行為が、既に一大産業になっているといえます。以前、田舎
にいたときには余り気がつきませんでしたが、ここ華南地域では、社会制度と
して(もっと有態にいえば官民あわせて)組織的に整備されているといってもい
い状態です。
そのような中ですら、コンドームの使用は個人に任されているようです。
エイズの脅威は知っていても、有効な手段を持たない、貧しい多くの農村出身
小姐にはどうすることもできないのです。中国の中央政府レベルでその重い腰
をやっとあげつつありますが、地方の実態はエイズ予防という観点からは悪化
の一途であるともいえると思います。
同性保護という観点から授業で取り組まれることも良いと思います。

はい！授業でやりました。恥ずかしがりの男生徒は後ろでニヤニヤしながら、
意外に静かにしていましたね。みんな二十歳を過ぎたＹｏｕｎｇ Ａｄｕｌｔ
なのですが、精神年齢は低いようです。性教育を受けたことがあるかどうか、
をまず聞きましたが、ほとんどなし！
今中国では、大学生に性教育をするべきかどうかということが取り上げられて
います。私は、小学校でも早くないと思うのですが、、、。
親も教師も教えてくれない、で、性に関する知識は、雑誌や医学書、インター
ネットからの情報、友達との雑談から得るようです。コンドームを何のために
使うのかは知っていても、どうやって使うのかを知らないのではないでしょう
か。
私は実物を見せました。封を切って、バナナか人参にかぶせて実践をするつも
りでしたが、ちょっと気が引けて、「個人的に教えて欲しい人は、オフィスま
で来て！」とだけ言いました。」
性的活動が活発なのは、大学生頃の年齢です。それを中国の昔からのしきたり
に囚われて、結婚まではセックスなんて絶対だめ！と禁じることは無理でしょ
う。今、ＨＩＶエイズの感染は、大学生がハイリスクグループの中に入ってい
ると聞きます。
オープンに話し合うか、あくまでも保守的に性をベールで覆ったままにするか
難しい選択です。でもこのまま、見て見ぬふり、自分はエイズなんか関係ない
よ！では済まされない状態になっているのです。
周りの大人ー両親や親戚、教師も大学生の性を認めて、望まぬ結果にならない
ようにするべきです。
望まぬ結果とは、エイズを初めとする性感染症、妊娠と堕胎ということです。
そうなる前に、まず予防ですよね。まだ日本もそうですが、女性は受身です。
コンドームをつけて！と言うのをためらう人が多いということと、堕胎がたや
すくできること、この二つが、女性を傷つけています。
中国では、それに加えて貧困がどんどん女性のＨＩＶエイズの感染を悪化させ
ています。女性の保護、男性の意識改革が必要だと思います。生徒には、分か
りやすい言葉で、「愛情があってセックスをするのなら、お互いを大切にする
関係になりたいね」と言うつもりです。

Yes, I did in class what I thought was right. Shy boys
were quiet at the back of the classrooms with a grin on
their faces. College students are all young adults, but in
China they seem to be mentally immature. When asked
whether they had a sex education, most said NO.
At present, there is a hot debate on whether introducing a
sex education in college or not. Personally, I suggest
this should be done in primary school. In China, parents
and teachers do not teach young people about sex, and as a
result, most students rely on the magazines, medical
books, the Internet and friends for sex-related
information. They may know the purpose of condoms, but do
not know how to use them. I showed them one in class, but
a little hesitated to demonstrate the actual usage. I just
told them to come to my office in person if interested.
College students are sexually active, and it is not a good
idea to forbid sex just because of the Chinese
conventional idea of fidelity. Currently college students
are listed as one of the high risk group of STD infection.
It is a tough choice between discussing sex openly and
covering sex with a veil. However, the HIV/AIDS situation
in China is compelling people to face the reality. Adults
that include parents, relatives and teachers should admit
‘student sex’ and promote ‘safe and protected sex’.
As a result of ‘unsafe, unprotected and casual sex’,
women in general suffer from STD ( HIV is one of them)
infections and abortion. What can be done before it is too
late? Yes, prevention is better than cure. Women are
passive and usually the victims. Both Chinese and Japanese
women are still hesitant to ask men to put on condoms. It
is relatively easy to get an abortion. These two factors
make women vulnerable.
Additionally, in China poverty has worsened the female HIV
infections. Protecting the women
’s reproductive health and revolutionalizing men’s
awareness are vital for solving this problem. I just said
with a simple expression,
“If you decide to make love, establish an equal
relationship that enhances mutual love, care and respect.”
